Reporting to the Director, Clinical Services, and in cooperation with the patient/family and other professionals, the Pharmacist designs, implements and monitors medication therapy specific to the patients' needs. The Pharmacist identifies, resolves and prevents medication related problems and contributes to positive patient outcomes. The Pharmacist works with and supports the centralized dispensary computerized drug distribution system to ensure medications reach the patient in a safe and timely manner. All duties are performed in accordance with the Mission and Values of Providence Healthcare.

MAJOR RESPONSIBILITIES:

  • Complete medication reconciliation upon admission, transfer and discharge by creating the Best Possible Medication History (BPMH) utilizing medication from all sources (acute care/home/other facilities/Drug Profile Viewer);
  • Utilize the medication profile and patient, and medication-related information consistent with the principles of pharmaceutical care to:
    • Assess prescribed medications;
    • Monitor adherence to medication regimen;
    • Identify and assess the type and significance of medication-related problems, and discuss with the physician and/or care team;
    • Take appropriate intervention;
    • Dialogue with the patient/family as appropriate;
    • Document in a readily retrievable format any interventions and/or dialogue;
  • Participate in the interdisciplinary care planning process as a member of the patient care group;
  • Develop/review policies related to clinical practice;
  • Comply with professional practice standards consistent with the Regulated Health Professions Act (RHPA), the Pharmacy Act and the regulations hereunder;
  • Participate in formal presentation of patient cases (alone or with other members of the team) to general staff;
  • Promote the advancement of the profession through publication and other professional activities;
  • Provide education to physicians, nurses and other health professionals, patients and families, the community served by Providence Healthcare; act as a resource person for drug information questions, in-services, grand rounds, etc.;
  • Provide orientation and training for new staff members;
  • Participate as preceptors in the University of Toronto, Leslie Dan Faculty of Pharmacy, Early Student Exposure Program, and/or Advanced Pharmacy Practice Experience (APPE) program; and/or participate as preceptors with the School of Pharmacy, University of Waterloo, and the Ontario College of Pharmacists, training and internship programs.
  • Participate in pharmacy-related and collaborative interdisciplinary research;
  • Participate in continuous quality improvement activities;
  • Collaborate with the Director, Clinical Services to identify and develop ideas/initiatives to promote efficient operation or medication safety;
  • Monitor for and provide guidance on any practices that increase operational or drug costs.

 

QUALIFICATIONS/SKILLS REQUIRED:

  • Bachelor of Science in Pharmacy degree or entry-to-practice Doctor of Pharmacy degree from a recognized Faculty/College of Pharmacy, Pharmacy Examining Board of Canada (PEBC) designation and eligibility for licensure to practice in Ontario are required;
  • Minimum one (1) year experience of the pharmaceutical care model of practice in a community or hospital setting. Hospital experience is preferred;
  • A licensed incumbent should be able to show her/his learning portfolio as required by the Ontario College of Pharmacists. Membership in professional associations is preferred;
  • Demonstrate excellence in clinical practice, preferably in rehab/complex continuing setting;
  • Excellent interpersonal and communication skills, basic administrative skills;
  • Very good attendance record;
  • Good performance record.
