Key Duties and Responsibilities
- Respond to the needs of patients who present with undifferentiated and undiagnosed conditions.
- Attend to the needs of patients, presenting in a variety of ways. These include;
Secondary Triage of AMPDS Green coded incidents in accordance with UCO Policy
Identification of Urgent/Green GP or HCP admissions for secondary triage in accordance with UCO Policy
Secondary and Tertiary triage and assessment of higher priority incidents in accordance with escalation policies
- Provide assessment, treatment and diagnosis at point of first contact by attending to patients in a variety of clinical & non-clinical settings according to patient needs.
- Accurately triage & prioritise patients in strict adherence with use of the Manchester Triage System Telephone Triage and Assessment (MTS TTA) and variety of techniques to elicit the history of an event/illness including past medical and drug history.
- Evaluate clinical information from examination & history taking and initiate appropriate treatment and/or referral.
- Signpost appropriately to other services in the identified healthcare pathway.
- Refer patients to appropriate care settings, where appropriate.
- Discharge or treat, as appropriate and within scope of practice.
- Supply and/or administer medication and treatments according to agreed protocols and NWAS Trust Formulary.
- Follow national protocols and local guidelines.
- Maintain accurate and systematic documentation of patients' interactions and clinical care provided, including any adverse events, child protection issues or other notifiable matters.
Person Specification
- Member of the Health Professions Council (HPC) or member of the Nursing & Midwifery Council (UK)
- Registered Paramedic or General Nurse
- Level 6 Academic Module (related to clinical assessment) or commitment to complete within the stipulated academic timescales
- Emergency Care Practitioner/Nurse Practitioner (or equivalent) or Level 6 Telephone Triage and Assessment Module
- Formal mentoring qualification
- Appropriate post registration experience
- Excellent knowledge of Pre-hospital Care Provision
- Excellent verbal and written communication skills
- Good organisational skills
- Evidence of CPD
